I understand that this application cannot be assessed unless all the relevant sections of this form have been completed and all the required documents are attached.

I declare that the information given in this application form is complete, true and correct.

I understand that providing false or misleading information, or non-disclosure of relevant information, may result in cancellation of the participants enrolment.

I agree that if any of the information on this form changes, I will provide updated information to ACC as soon as possible and within 7 days at most.

Attendance Policy

It is the Participants obligation to commit to the training schedule either as advertised or as agreed upon prior to the start of the training or coaching.

Agile Career Consulting would make conscious efforts to accommodate rescheduling a maximum of two missed classes in the upcoming training workshop as long as ACC is notified at least twenty-four hours prior to the start of the class.

Please not that immediate rescheduling in an upcoming training workshop is not guaranteed, as precedence would be given to participants who are already registered for the next training workshop. Also ‘No shows’ are ineligible for rescheduling and would be considered a forfeit.

By signing this agreement, you fully understand the expectations of ACC’s training policy which includes and not limited to the rights to include your picture/videos and any other material as marketing content for the company. Images acquired during training sessions are properties of ACC and content(s) may be used for marketing.

Contract Agreement

This agreement for training and coaching services is between The Participant and Agile Career Consulting (ACC). By making payment, you agree to our policies below:

ACC will provide interactive online training to registered participants only when payments have been made in full
ACC will provide you with services according to the service package you selected and paid for as all services vary and are sometimes customizable to fit our needs
ACC Does Not guarantee any job placement, however we will guide you and provide you with the necessary skills, coaching, and preparation (in accordance with the service package you paid for) to assist you in landing the ideal job
You are required to make all payments in full prior to the start of the training, unless a flexible payment structure was agreed upon
All fees must be paid as directed in other for ACC to provide the requested services, as all deposit and service fees are non-refundable. Once payment has been received and services have commenced, refunds would not be processed

Acknowledgement and Consent
